Collections of personal information
Personal information will be collected and used in a responsible manner and only to the extent necessary for the services provided. Information obtained is held strictly confidential. Journey to Health is obligated to retain your personal health information for a minimum of 10 years after a client’s last contact with clinicians.
Please note, we do not consider email to be a secure form of communication. While we use email periodically for scheduling appointments or sending resources, we advise that you avoid communicating sensitive information through email. Any sensitive information should be communicated during your session with your clinician (Therapist and/or Registered Dietitian).
Limits of Confidentiality
Journey to Health will only collect, use or disclose your personal health information with your express or implied consent, unless a collection, use or disclosure without consent is permitted or required by law.
Therapists and Registered Dietitians at Journey to Health may be required by law to disclose your personal health information in any of the following circumstances:
a) There is an imminent danger to an identifiable third party or to self; b) When there is reasonable cause to suspect abuse or neglect of a child; c) When a disclosure is ordered by a court; d) When a client requests disclosure, or e) When a client files a complaint or claims professional liability by the health care provider in a lawsuit.
